Anxiety and Cognitive Decline in Older Adults
Summary
Anxiety is increasingly recognised as both a contributor to and a harbinger of cognitive decline in later life. In community and clinical settings, elevated anxiety symptoms and diagnosed anxiety disorders have been linked to poorer performance in critical cognitive domains such as memory, processing speed and executive function. Several biological mechanisms have been proposed. Chronic anxiety is associated with dysregulation of the hypothalamic–pituitary–adrenal axis and accompanying elevated cortisol levels, which may impair hippocampal integrity. Anxiety also correlates with heightened systemic inflammation and vascular dysfunction, both established drivers of neurodegeneration. Behavioural pathways are equally important: persistent worry and hypervigilance can erode cognitive reserve by reducing engagement in mentally stimulating activities and by fragmenting attention. From a public health perspective, anxiety represents a potentially modifiable risk factor; early identification and effective treatment may not only alleviate psychiatric distress but also mitigate the trajectory of cognitive deterioration. Psychological interventions, pharmacological strategies and lifestyle modifications all show promise in bolstering cognitive resilience among anxious older adults. Understanding the complex interplay of affective symptoms and neurocognitive processes is vital for designing targeted prevention and rehabilitation programmes that preserve autonomy and quality of life.
