Apathy Assessment and Intervention in Neurocognitive Disorders

Summary

Apathy in neurocognitive disorders such as Alzheimer’s disease, frontotemporal dementia, Parkinson’s disease and cerebrovascular syndromes is characterised by diminished initiative, goal-directed behaviour and emotional engagement. It affects up to half of patients across these conditions, worsening functional outcome, increasing caregiver burden and hastening cognitive decline. Assessment typically combines informant-based questionnaires and clinician-rated scales to capture reductions in motivation, cognition and social interaction. Neuroimaging studies implicate disruption of frontostriatal circuits, particularly the dorsal anterior cingulate cortex and ventral striatum, and emerging diffusion and functional connectivity analyses highlight widespread white-matter network disconnection as a substrate. Interventions under investigation include pharmacological agents targeting dopaminergic and cholinergic systems, structured behavioural activation programmes and non-invasive neuromodulation techniques. Although modest benefits have been reported, heterogeneity in patient subtypes and outcome measures underscores the need for personalised approaches and mechanistically informed trials.
