Summary

Arachnoid cysts are benign extra-axial cerebrospinal fluid (CSF) collections enclosed by arachnoid membranes, representing approximately 1 % of intracranial lesions. They are most frequently located in the middle cranial fossa but may arise in suprasellar, posterior fossa or quadrigeminal regions. Many remain asymptomatic and are detected incidentally through magnetic resonance imaging (MRI) or computed tomography (CT), but larger or strategically positioned cysts can exert mass effect, hydrocephalus or cognitive disturbances. MRI remains the diagnostic gold standard, employing T1, T2 and fluid-attenuated inversion recovery sequences; advanced modalities such as diffusion tensor imaging and phase-contrast CSF flow studies enrich lesion characterisation and surgical planning. Ultrasonography serves as a rapid screening tool in infants. Management strategies span conservative monitoring with serial imaging for stable cysts to surgical intervention for symptomatic or enlarging lesions. Surgical techniques have evolved from open microsurgical fenestration and shunting procedures to minimally invasive neuroendoscopic fenestration, often aided by neuronavigation and intraoperative imaging to enhance safety. Such tailored approaches seek to mitigate mass effect, prevent rupture or haemorrhage and optimise neurocognitive outcomes. Globally, research efforts are refining risk stratification, especially in paediatric and athletic populations, and advancing standardised management protocols through multidisciplinary collaboration.
