Summary

Aristolochic acids are a group of plant‐derived nitrophenanthrene carboxylic acids known to cause severe renal injury and urothelial carcinoma. Historically implicated in traditional herbal remedies and environmental contamination, these compounds provoke a distinct form of interstitial nephritis characterised by progressive tubular atrophy, interstitial fibrosis and a high risk of upper tract urothelial malignancies. Two main clinical entities arise from chronic exposure: aristolochic acid nephropathy, often observed in individuals self‐medicating with herbal products, and Balkan endemic nephropathy, restricted to populations consuming AA‐contaminated grain. Pathogenesis involves enzymatic activation of AA, formation of DNA adducts, persistent mutational stress and dysregulation of vasoactive mediators, culminating in irreversible renal scarring. Despite widespread bans, ongoing global use of AA‐containing plants and under‐recognition of exposure perpetuate the disease burden. Emerging insights into molecular mechanisms and biomarker development are vital for improved diagnosis, risk assessment and therapeutic innovation.
