Summary

The assessment of physical activity in older adults encompasses a range of methods to capture frequency, intensity, time and type of movement across daily life. Accurate measurement is critical to understand the relationships between activity patterns, functional capacity, risk of chronic illness and quality of life. Traditional self-report instruments offer cost-effective means to gather information on structured and unstructured activity but are subject to recall bias and may under- or overestimate true activity levels. Objective methods—principally accelerometry—provide fine-grained data on movement intensity and duration yet require standardisation of device placement, epoch length and intensity cut-points to ensure comparability. Emerging approaches integrate wearable sensors with smartphone applications and machine-learning algorithms to classify activity types, transitions and sedentary bouts. Global surveillance efforts increasingly recommend harmonised protocols to facilitate cross-national comparisons and to inform guidelines on light, moderate and vigorous activity thresholds. In practice, a combination of subjective and objective measures is often deployed to balance feasibility with validity, enabling clinicians and researchers to tailor interventions to promote healthy ageing and maintain independence in later life.

Moderate-to-vigorous physical activity (MVPA): Activity performed at an intensity that raises heart rate to 55–70% of maximum, typically including brisk walking, cycling or swimming.

Sedentary behaviour: Waking activities characterised by energy expenditure ≤1.5 metabolic equivalents, such as sitting, reclining or screen time.

Accelerometry: Objective measurement using wearable motion sensors that record acceleration signals to infer activity intensity and duration.

Epoch length: The time interval over which accelerometer data are summarised, commonly set at 10- to 60-second windows to capture movement patterns.

Intensity cut-points: Thresholds of accelerometer counts per minute used to classify activity as light, moderate or vigorous intensity.
