Summary

Auditory processing deficits encompass a spectrum of conditions arising from disruption to the central auditory system, typically located within the temporal lobes and related cortical and subcortical pathways. These disorders range from cortical deafness—complete loss of sound perception despite intact peripheral hearing—to selective impairments such as pure word deafness and auditory agnosia, in which speech or environmental sounds fail to be recognised. Underlying causes include stroke, trauma, neurodegeneration and rare vascular pathologies. At a structural level, damage to the primary auditory cortex in Heschl’s gyrus, the superior temporal cortices and connecting white matter tracts such as the acoustic radiation can produce diverse clinical phenotypes. Recent advances in neuroimaging, electrophysiology and behavioural assessment have refined our understanding of the hierarchical organisation of auditory processing, the lateralisation of language functions and the plasticity of compensatory networks. Clinically, precise characterisation of auditory cortical disorders informs rehabilitation strategies and underpins early intervention in ageing and cerebrovascular populations.

Advances in diffusion MRI tractography have enabled detailed in vivo mapping of the acoustic radiation, the thalamo-cortical fibre bundle conveying sound information from the medial geniculate nucleus to the auditory cortex. Through optimisation of acquisition parameters and probabilistic tracking algorithms, researchers have generated a high-resolution stereotaxic atlas of the acoustic radiation that reveals consistent patterns of left-hemispheric lateralisation. This atlas supports reliable reconstruction of individual white matter pathways and establishes normative benchmarks for assessing disconnection in cortical auditory disorders. Such methodological foundations pave the way for integrated studies of structural damage and functional outcomes in conditions ranging from stroke-induced cortical deafness to developmental central auditory processing disorders.
