Recent studies have demonstrated that combining custom-made biodegradable scaffolds with a period of prevascularisation followed by delayed fat injection can regenerate substantial volumes of adipose tissue. In a large‐animal model, three-dimensional polycaprolactone constructs were implanted and allowed to vascularise for two weeks before lipoaspirate was introduced. After 24 weeks, the prevascularised group exhibited adipose tissue formation comparable in volume and histological architecture to native tissue, outperforming both immediate fat injection and scaffold‐only controls. This approach provides a proof of principle for reconstructing large soft tissue defects with enhanced vascular integration and stable volume retention.
