Summary

The landscape for early detection and staging of hepatocellular carcinoma (HCC) has evolved with the integration of serum, cellular and imaging biomarkers that offer non-invasive, cost-effective strategies. Alpha-fetoprotein (AFP) remains the most widely adopted marker, yet its limited sensitivity for early lesions and false-positive rates in chronic liver disease have driven the search for complementary indicators. Emerging protein-based markers such as prothrombin induced by vitamin K absence or antagonist-II (PIVKA-II) and tumour-associated antigens have demonstrated enhanced diagnostic accuracy when combined with AFP and imaging modalities. Cellular assays measuring parameters such as monocyte distribution width (MDW) capture host-immune responses and may reflect underlying inflammation and tumour burden. Moreover, advances in biostatistical analysis, including receiver operating characteristic curves and machine-learning algorithms, have refined threshold determination and multiplex panels to yield higher sensitivity and specificity. Integrating multi-marker panels into routine surveillance protocols has shown promise in stratifying patients according to risk, guiding therapeutic decisions and monitoring recurrence. The continued validation of novel biomarkers and harmonisation of assay standards remain pivotal to translating these methods into global clinical practice.
