Summary
Human skeletal integrity is intimately linked to the balance of lean and fat mass, with metabolic disturbances such as obesity, insulin resistance and type 2 diabetes exerting complex influences on bone health. Mechanical loading from muscle mass generally promotes bone formation, whereas excessive adiposity—particularly visceral fat—can impair bone quality through pro-inflammatory cytokines and dysregulated adipokine secretion. Subcutaneous fat and stable weight trajectories may exert protective effects, but rapid or late-life weight gain often accelerates site-specific bone loss. Novel anthropometric measures reveal non-linear associations between fat distribution and bone mineral density, emphasising the need for refined risk assessment and targeted interventions across the life course.
