Burn Injury Rehabilitation and Contracture Management
Summary
Burn injury rehabilitation encompasses a multidisciplinary process aimed at restoring form and function following thermal trauma. From the acute phase through long-term recovery, the focus lies on preserving joint mobility, minimising scar formation and promoting psychosocial wellbeing. Early intervention begins with positioning, pressure therapy and splinting to limit the onset of contractures—pathological tightening of skin and underlying tissues that restrict movement. As wounds heal, active and passive stretching, targeted exercises and occupational therapy support the maintenance of range of motion (ROM) across affected joints. In parallel, reconstructive techniques such as autologous skin grafts and various flap surgeries address established contractures, replacing rigid scar tissue with pliable, well-vascularised tissue. Emerging modalities—in particular virtual reality–based rehabilitation—have demonstrated promise in engaging patients and enhancing upper limb function. Rehabilitation goals extend beyond physical recovery to encompass quality of life, with psychological support and patient education integral to long-term outcomes. Effective management reduces morbidity, expedites return to daily activities and mitigates healthcare burdens globally, especially within resource-limited settings where delayed access can exacerbate functional impairment.

A recent cross-sectional analysis examined the prevalence and predictors of scar contracture-associated re-hospitalisation among several hundred thousand burn inpatients. It revealed that nearly 1 per cent required further admission for contracture release, with higher risk observed in younger males, blue-collar workers, those with extensive burns (≥ 40 per cent total body surface area) and cases involving surgical treatment. These findings highlight the enduring clinical and economic impact of contractures and underscore the need for targeted preventive strategies and early rehabilitation protocols to reduce re-hospitalisation rates.

Contracture: Pathological shortening of skin, muscle or connective tissue leading to restricted joint movement.
Range of Motion (ROM): The degree of movement at a joint, often measured in degrees of flexion and extension.
Autologous Skin Grafting: Transplantation of a patient’s own skin from a donor site to cover a burn wound or scar defect.
Flap Surgery: Reconstruction technique transferring tissue (skin, fat, muscle) with its blood supply to a defect, often used to release contractures.
Virtual Reality–based Rehabilitation: Use of immersive computer-generated environments to deliver interactive therapeutic exercises that enhance motor recovery.
