Summary

Cancer epidemiology seeks to quantify the distribution and determinants of malignant disease across populations, informing strategies for prevention, early detection and treatment. Central metrics include incidence (new cases), prevalence (existing cases) and mortality. Age‐standardised rates allow comparison across regions with differing demographic structures, while measures such as disability‐adjusted life years (DALYs) and years of life lost (YLLs) capture the broader health impact. Advances in electronic registry systems and statistical modelling have improved the timeliness and granularity of data, revealing marked variations by geography, socioeconomic status and tumour site. These variations reflect underlying exposures—tobacco, infection, environmental pollutants and lifestyle transitions—and access to screening and care. Cancer burden assessment integrates these metrics to identify priority areas for intervention, to evaluate the effectiveness of control programmes and to project future needs. Globally, the rising incidence of cancers associated with ageing populations and “westernised” risk factors now coexists with persistent burdens of infection‐related and digestive tract malignancies in many low‐ and middle‐income settings, underlining the need for tailored policies spanning tobacco control, vaccination, early detection and equitable treatment access.

A population‐based age–period–cohort analysis of oesophageal and gastric cancer mortality in China disentangled temporal trends across birth cohorts and calendar periods, revealing stronger declines in oesophageal cancer mortality in rural areas consistent with the impact of screening initiatives. Cohort patterns for both cancers exhibited an inverse U-shape, implicating early‐life nutritional deficits in the mid-20th century as a shared aetiological pathway. Findings support expansion of targeted screening for gastric cancer and strengthened nutritional and environmental interventions in vulnerable cohorts.

Incidence rate: Number of new cancer cases per unit population in a given time, often age-standardised to allow comparisons.

Mortality rate: Number of deaths due to cancer per unit population over a specified period, usually age-standardised.

Age–period–cohort analysis: Statistical modelling that separates effects due to a person’s age, time period and birth cohort to elucidate trends.

Disability‐adjusted life year (DALY): Composite measure of disease burden combining years of life lost due to premature death and years lived with disability.

Burden assessment: Integrated evaluation of incidence, mortality, survival and quality-of-life metrics to guide public health policy and resource allocation.
