Carnitine Supplementation and Sperm Quality in Reproductive Biology

Summary

Carnitine, a naturally occurring quaternary ammonium compound, plays a central role in the transport of long-chain fatty acids into mitochondrial matrices, thereby facilitating β-oxidation and ATP generation. In the male reproductive tract, high concentrations of carnitine in the epididymal lumen support sperm maturation, motility and membrane integrity. Supplementation with L-carnitine has been explored as a strategy to enhance sperm function by bolstering energy metabolism and quenching reactive oxygen species. Studies spanning in vitro culture of human and animal sperm to in vivo models of metabolic stress have demonstrated that appropriate doses of L-carnitine can improve motility, viability, mitochondrial activity and zona pellucida binding. These effects have direct implications for assisted reproduction technologies, semen preservation and therapeutic intervention in subfertile males. Emerging research emphasises optimisation of dosage, timing and delivery medium to maximise the protective and bioenergetic benefits of carnitine while avoiding potential pro-oxidant or osmotic imbalances.
