Case-Based Learning in Health Professional Education

Summary

Case-Based Learning (CBL) has emerged as a cornerstone of contemporary health professional education, shifting the balance from passive receipt of information to active, contextualised engagement with clinical scenarios. By presenting learners with authentic patient cases, CBL fosters clinical reasoning, integration of theoretical knowledge with practice and the development of key competencies such as communication, teamwork and ethical decision-making. Within small-group settings, students analyse case histories, interpret diagnostic data and propose management plans under faculty facilitation. This approach mirrors real-world clinical environments, encourages self-directed inquiry and cultivates reflective practice. Globally, CBL has been adapted across diverse health disciplines—medicine, nursing, biomedical science and allied health—often in combination with complementary pedagogies such as Problem-Based Learning (PBL), team-based learning and outcome-centred models. Technological enhancements, including e-learning platforms and virtual patient simulations, have extended the reach of CBL to hybrid and fully online contexts, ensuring continuity of training in settings constrained by resources or physical distancing. Evidence consistently demonstrates that CBL strengthens critical thinking, improves examination performance and enhances learner satisfaction, while also nurturing interprofessional collaboration and adaptability to evolving healthcare demands.

Research from Nature Portfolio

Recent studies have explored the integration of CBL with structured teaching frameworks to enhance student engagement and learning outcomes. One investigation applied a combined PBL-CBL approach to the teaching of international classification of diseases (ICD) encoding for health information management students. The blended model yielded higher theoretical knowledge and practical coding skills, alongside marked gains in learners’ abilities to acquire, share, store and apply knowledge. A parallel enquiry introduced a modular teaching design that married the BOPPPS model (Bridge-in, Objectives, Pre-assessment, Participatory Learning, Post-assessment and Summary) with the principles of Outcome Based Education (OBE) in a clinical biochemistry course. This reform aligned course objectives with professional competencies across knowledge, skill and attitudinal domains and demonstrated significant improvements in student engagement, examination performance and problem-solving proficiency compared with traditional methods.

The graph below shows the total number of articles in case-based learning in health professional education across all publications each year (not limited to Nature Index journals).

Technical terms

Case-Based Learning (CBL): An instructional method in which learners work through authentic clinical cases to develop problem-solving and decision-making skills.

Problem-Based Learning (PBL): A student-centred pedagogy that uses complex, real-world problems as the stimulus for learning foundational knowledge and skills.

BOPPPS model: A structured teaching framework comprising Bridge-in, Objectives, Pre-assessment, Participatory Learning, Post-assessment and Summary steps to guide lesson design.

Outcome Based Education (OBE): An educational approach focused on achieving specified competencies and measurable learning outcomes aligned with professional requirements.

Objective Structured Clinical Examination (OSCE): A performance-based assessment in which learners demonstrate clinical skills and reasoning through timed stations with standardised tasks or simulated patients.
