Summary

Cauda equina syndrome (CES) is a rare but urgent neurological condition arising from compression of the lumbosacral nerve roots within the spinal canal. Clinical presentation typically includes bilateral sciatica, saddle sensory disturbance, lower limb motor weakness, and autonomic dysfunction manifesting as urinary retention, faecal incontinence or sexual impairment. Prompt recognition relies on detailed history, neurological examination and imaging—magnetic resonance imaging remains the gold standard for visualising nerve-root compression. Measurement of post-void residual volume can aid assessment of bladder function but should not delay definitive imaging when clinical suspicion is high. Management centres on emergency surgical decompression, most commonly via laminectomy or discectomy, to relieve mechanical pressure and preserve neurological function. Post-operative care involves multidisciplinary rehabilitation and long-term monitoring of bladder, bowel and sexual recovery. Outcomes vary according to severity at presentation, duration of compression and completeness of initial dysfunction; earlier decompression generally yields better functional recovery though residual deficits in micturition and continence are common. Standardisation of definitions and outcome measures is crucial to optimise care pathways, minimise medicolegal risk and improve patient counselling regarding prognosis and quality of life.
