Adjunctive corticosteroids reduce mortality by mitigating inflammatory damage, particularly in cases of meningeal inflammation and hydrocephalus. Surgical interventions, such as ventriculoperitoneal shunting, may be required for refractory hydrocephalus, while antiplatelet or anticoagulant therapies are under investigation for preventing cerebral infarction. Emerging drug-resistant strains, immune reconstitution inflammatory syndrome and HIV co-infection complicate management and underscore the need for novel therapeutics and tailored regimens. A multidisciplinary approach involving neurologists, infectious-disease specialists and neuroradiologists remains pivotal to improving outcomes in this high-mortality condition.
