Cerebellar Liponeurocytoma and Central Nervous System Tumor Classification

Summary

Cerebellar liponeurocytoma is a rare, generally benign neoplasm arising in the posterior fossa, characterised by neurocytic cells with focal lipomatous differentiation and variable astrocytic features. First distinguished in the World Health Organization (WHO) classification of central nervous system tumours, it is designated as a grade II lesion owing to occasional recurrences and a low but recognisable proliferative index. The entity predominantly affects adults in mid-life, presenting clinically with headaches, ataxia or signs of raised intracranial pressure. Radiologically, magnetic resonance imaging often reveals a well-circumscribed mass with heterogeneous signal intensities reflecting both neural and fatty components. Histopathological diagnosis relies on demonstration of synaptophysin and neuron-specific enolase positivity, a low Ki-67 labelling index and ultrastructural evidence of lipid-laden tumour cells. In the broader schema of CNS tumour classification, cerebellar liponeurocytoma occupies a distinct niche among neuronal and mixed neuronal–glial tumours, warranting differentiated management from more aggressive entities such as medulloblastoma. Complete surgical resection remains the cornerstone of treatment, with adjuvant radiotherapy reserved for residual or recurrent disease. Ongoing classification efforts continue to refine its relationship to other posterior fossa neoplasms and to integrate molecular markers that may predict clinical behaviour.
