Summary
Multiple sclerosis (MS) is an immune‐mediated disorder of the central nervous system that frequently poses diagnostic challenges, particularly in early or atypical presentations. Analysis of cerebrospinal fluid (CSF) offers direct insight into intrathecal immune activity and axonal injury. The classical hallmark in MS diagnostics is the detection of immunoglobulin G oligoclonal bands (OCB), which signal persistent B-cell activation within the central nervous system. More recently, quantitative measurements of kappa free light chains (κ-FLC) have emerged as a rapid and standardised alternative to OCB, reflecting overall immunoglobulin synthesis. Parallel advances include the measurement of neurofilament light chain (NfL) as a marker of neuronal damage and the use of paired CSF‐serum indices to distinguish central from peripheral contributions. Together, these biomarkers enhance diagnostic sensitivity, aid in prognostic stratification and support therapeutic decision-making. Their incorporation into revised diagnostic criteria has global implications, reducing misdiagnosis and enabling earlier intervention. Moreover, ongoing work seeks to integrate biomarker panels for personalised risk assessment, treatment monitoring and prediction of long-term outcomes, thereby reinforcing the role of CSF analysis as an indispensable tool in contemporary MS care.
