Summary

Chemokines are small secreted proteins that orchestrate the targeted migration of immune cells into and within the central nervous system (CNS). In autoimmune disorders such as multiple sclerosis (MS), a dysregulated chemokine network contributes to the breach of the blood–brain barrier, the recruitment of autoreactive lymphocytes and monocytes, and the amplification of neuroinflammatory cascades. Members of the CC and CXC chemokine families form gradients that direct cell subsets expressing cognate receptors, thereby shaping lesion distribution, severity of demyelination and neurodegeneration. Within the CNS parenchyma, resident glial cells also produce chemokines in response to injury or cytokine signals, creating local amplification loops that sustain inflammation. Temporal profiling has demonstrated that early surges in certain chemokines correlate with active demyelination, while others persist in chronic lesions, influencing remyelination and tissue repair. Advances in imaging, fluid biomarker analysis and single-cell transcriptomics are now revealing the spatial and cellular specificity of chemokine expression, informing both mechanistic understanding and therapeutic targeting. Modulation of chemokine–receptor interactions holds promise for limiting pathologic immune cell trafficking without broadly suppressing systemic immunity.
