Chronic Kidney Disease Implications in Type 2 Diabetes Management
Summary
Chronic kidney disease (CKD) represents one of the most serious microvascular complications of type 2 diabetes mellitus, affecting renal structure and function through mechanisms that include glomerular hyperfiltration, oxidative stress and low-grade inflammation. Progressive decline in renal function compounds cardiovascular risk, necessitates adjustment of glycaemic targets and demands careful selection and dosing of antihyperglycaemic and antihypertensive therapies. Early detection through regular estimation of glomerular filtration rate and assessment of urinary albumin excretion enables risk stratification, facilitates renoprotective interventions and may slow progression towards end-stage kidney disease. Integrating multidisciplinary care—encompassing tight control of blood pressure, optimisation of glycaemic control, lipid management and lifestyle modification—reduces both renal and cardiovascular morbidity. Recent therapeutic advances, notably the introduction of sodium–glucose cotransporter 2 inhibitors and non-steroidal mineralocorticoid receptor antagonists, offer significant renoprotection beyond glucose lowering. Globally, disparities in access to screening and treatment underscore the need for tailored public health strategies, especially in regions with high diabetes prevalence. In clinical practice, CKD in people with type 2 diabetes demands vigilant monitoring, timely therapeutic adaptation and close collaboration between diabetologists and nephrologists to achieve optimal outcomes.

Analyses of a large Southeast Asian diabetes registry have revealed that over half of patients with type 2 diabetes exhibit evidence of diabetic kidney disease, defined by reduced filtration rate and/or albuminuria. Age, duration of diabetes, suboptimal glycaemic control and hypertension emerged as the most significant risk factors, highlighting the urgent need for integrated monitoring programmes in primary care. A complementary study in a northern Thai cohort of more than a thousand primary-care patients reported a CKD prevalence of nearly one quarter, with higher stages seen in older individuals and those with poor glycaemic indices. The work underscores that early identification of albuminuria and timely initiation of renin–angiotensin system inhibitors can modify disease trajectory, improving long-term renal and cardiovascular prognosis.
