Summary
Human physiology is governed by an intrinsic circadian rhythm that synchronises sleep, hormone secretion and metabolism with the external day–night cycle. Shift work, by imposing schedules that conflict with this endogenous clock, gives rise to circadian misalignment and a cascade of adverse outcomes. Commonly observed consequences include impaired sleep quality, metabolic disturbances, elevated risk of cardiovascular disease, mood disorders and weakened immune responses. Longitudinal evidence also links chronic shift work to higher incidence of certain cancers and neurocognitive decline. Individual differences in chronotype, light exposure and sleep behaviours influence the degree of adaptation, yet many workers remain only partially aligned even after prolonged exposure to night or rotating shifts. Countermeasures under investigation include optimised shift schedules, timed light therapy, pharmacological agents and behavioural strategies aimed at accelerating realignment. Understanding the mechanisms and variability of adaptation is key to reducing health burdens and improving occupational safety across diverse industries and geographical regions.

Recent studies have proposed that non-photic cues, notably physical activity, can act as supplementary zeitgebers to facilitate circadian adaptation in shift workers. A newly developed framework distinguishes adaptation across three timeframes—within individual shifts, across successive blocks of shifts and over an entire career—and suggests that targeted exercise interventions may modulate biomarkers of alertness, sleep quality and cognitive performance. Practical considerations include timing of activity relative to shift onset and interaction with light exposure, offering a cost-effective complement to existing strategies.
