Circadian Rhythm Dynamics in Aging and Cognitive Health
Summary
The circadian system orchestrates daily oscillations in sleep, hormone secretion, metabolism and cognitive performance through a central pacemaker in the suprachiasmatic nucleus and peripheral clocks in tissues throughout the body. With advancing age, the amplitude of these oscillations often diminishes and their phase relationships shift, leading to fragmented sleep–wake cycles and altered timing of peak alertness. Such changes can exacerbate age-related cognitive decline and predispose to neurodegenerative diseases by disrupting restorative processes, impairing synaptic plasticity and altering clearance of toxic metabolites. Maintenance of robust 24-hour rhythms has therefore emerged as a vital component of healthy brain ageing, with potential to inform interventions that stabilise sleep, enhance daytime activity and minimise circadian misalignment in older adults.

Recent studies have demonstrated that fine-scale patterns of motor activity provide sensitive markers of cognitive deterioration in dementia. In a long-term randomised trial of patients with Alzheimer’s disease, temporal correlations of activity at scales below two hours steadily weakened as clinical severity increased. Bright light therapy delivered in the morning attenuated this decline by nearly three-quarters and was accompanied by slower cognitive and mood deterioration. These findings suggest that multiscale analysis of activity fluctuations may offer an unobtrusive means to monitor disease progression and to evaluate the efficacy of chronotherapeutic interventions in real-world settings.
