Cleft Palate Surgical Techniques and Outcomes

Cleft palate repair encompasses a spectrum of techniques designed to close the palatal defect, reconstitute the muscular sling of the soft palate and restore velopharyngeal competence while preserving maxillofacial growth. Hard‐palate methods include the von Langenbeck two‐flap approach, V-Y pushback and vomerine flaps, each balancing access against tension on closure. Soft‐palate strategies centre on intravelar veloplasty, double-opposing Z-plasty and radical muscle dissection, all aiming to reorient and advance the levator veli palatini muscle to optimise speech outcomes. Timing typically falls between six and twelve months of age to minimise interference with feeding and language development. Key outcome measures are the rate of oronasal fistula, incidence of velopharyngeal insufficiency necessitating secondary surgery and long-term speech intelligibility. Complications such as flap necrosis, fistula formation and impaired maxillary growth remain challenges, driving innovations in preclinical tissue engineering, standardisation of outcome reporting and adjunctive diagnostic tools. Global variations in surgical scheduling and resource availability have prompted context-specific protocols, yet the overarching goal remains a one-stage repair that achieves functional closure, preserves growth trajectories and delivers normal resonance and articulation.
