Summary

Comorbidity between mood and anxiety disorders is highly prevalent and contributes to greater illness severity, chronicity and functional impairment than either condition alone. Clinical assessment requires a comprehensive approach that integrates categorical diagnoses with dimensional specifiers such as anxious distress or anxious depression. Standardised interviews and validated rating scales are essential for identifying overlapping symptom domains, distinguishing primary from secondary presentations, and monitoring treatment response. Management strategies combine pharmacological and psychological interventions tailored to the individual’s symptom profile and comorbid complexity. Selective serotonin reuptake inhibitors and serotonin–noradrenaline reuptake inhibitors remain first-line treatments, often supplemented by augmentation strategies and close monitoring for side effects. Cognitive behavioural therapy and transdiagnostic psychotherapies address shared cognitive and behavioural mechanisms, while collaborative care models and digital health tools enhance treatment access and patient engagement. Emerging biomarkers from neuroimaging and inflammatory studies promise to refine prognostic stratification and guide personalised interventions. Integrated care pathways that bridge primary and specialist services are critical for improving outcomes and reducing the global burden of these co-occurring disorders.
