Summary

Klebsiella pneumoniae is a Gram-negative bacillus responsible for a spectrum of illnesses ranging from community-acquired pneumonia to life-threatening sepsis and nosocomial outbreaks. Its capacity to acquire virulence factors, such as polysaccharide capsules, alongside mobile genetic elements encoding extended-spectrum β-lactamases and carbapenemases, has driven a global rise in multidrug-resistant strains. Epidemiological surveillance demonstrates variable incidence across regions, with higher burdens in settings lacking robust infection-control measures. Healthcare-associated infections frequently present in patients with comorbidities or indwelling devices, while hypervirulent lineages have re-emerged in the community, particularly in East Asia. Clinical outcomes are determined by host factors, site of infection and promptness of effective therapy. Mortality rates for bacteremia and pneumonia remain high, often exceeding 20–30 %, and climb further when resistance compromises empirical treatment. Emerging molecular tools for rapid pathogen detection, coupled with integrated “omics” analyses, are refining our understanding of host–pathogen interactions and guiding novel therapeutic and preventive strategies. Vaccines targeting key capsular antigens and monoclonal antibodies against surface components are under investigation. Global collaboration in surveillance, antimicrobial stewardship and clinical trials is essential to mitigate the threat posed by K. pneumoniae and to inform context-specific guidelines for diagnosis, treatment and prevention.

Recent studies have applied integrated genomic, transcriptomic, proteomic and metabolomic approaches to clinical isolates of K. pneumoniae, identifying conserved pathways activated during sepsis, including lipid-biosynthesis remodelling and mechanisms of osmotic adaptation in human serum. This multi-omics reference dataset offers a platform for target discovery and translational research. Foundational work has also illuminated how multidrug-resistant K. pneumoniae modulates virulence and resistance gene expression in response to co-pathogens, commensals and host cells. These experiments reveal species-specific interactions that influence antimicrobial susceptibility and suggest that manipulation of the local microbiome could become an adjunctive strategy in treating polymicrobial infections involving resistant K. pneumoniae.
