Summary

Since its emergence, COVID-19 has progressed in successive epidemiological waves, each marked by distinct clinical presentations, demographic profiles and outcomes. Initial waves were characterised by high rates of severe pneumonia, respiratory failure and mortality among hospitalised patients, particularly older adults with multiple comorbidities. Subsequent waves saw shifts towards younger cohorts, lower case fatality rates and milder disease, influenced by viral evolution, increasing population immunity and refined management protocols. Across settings, key drivers of mortality have included age, comorbidity burden, markers of systemic inflammation and respiratory compromise. Variations in health-care capacity, public health measures and vaccination coverage have further modulated clinical severity and resource utilisation. Understanding these dynamic features is critical to optimise patient care and inform policy responses during ongoing and future pandemics.
