Summary

Cochlear implants have transformed the management of severe to profound sensorineural hearing loss by directly stimulating the auditory nerve, restoring access to sound and speech. Outcomes extend beyond audiometric measures to encompass health-related quality of life, encompassing emotional well-being, social participation and functional independence. Although the majority of adult recipients experience marked improvements in speech perception, substantial variability persists, driven by factors such as duration of deafness, residual cochlear health, cognitive capacity and timing of intervention. Quality-of-life gains are demonstrable through generic and disease-specific instruments, revealing enhanced communication, reduced feelings of isolation and preserved mental health across age groups. Current research emphasises a holistic approach, integrating preoperative prediction tools, personalised rehabilitation strategies and systematic assessment of quality-of-life metrics to optimise patient selection, counselling and long-term support on a global scale.

Recent studies have investigated the influence of patient knowledge, attitudes and real-world practices on device use, showing that higher levels of cochlear implant literacy directly improve user expectations, care routines and device maintenance. Preoperative measures of cochlear substrate, obtained via electrocochleography total response, explain nearly half of the variability in speech perception in quiet and, when combined with cognitive screening, account for over 60 percent of performance in noisy environments. Foundational work utilising machine-learning algorithms has further refined outcome prediction models, demonstrating that shorter durations of deafness, younger age at implantation and sustained hearing-aid use until surgery yield the most accurate forecasts of postoperative word recognition, thereby underscoring the time-sensitive nature of implantation decisions.
