Summary

The study of cognitive abilities across the lifespan and their impact on health outcomes has progressed from descriptive epidemiology to integrated models that span prenatal development through to advanced age. Early-life cognitive function, often indexed by intelligence tests administered in childhood or adolescence, has been linked to long-term risks of mortality, chronic disease and mental health. This association appears to be graded: higher cognitive scores correlate with lower all-cause and cause-specific mortality, including cardiovascular disease, respiratory illness and dementia. Midlife cognitive trajectories and psychosocial factors—such as educational attainment, occupational complexity and social engagement—further shape late-life outcomes, suggesting multiple sensitive periods during which cognitive reserve can be fortified.

Recent decades have seen growing attention to the mechanisms mediating these associations. Socioeconomic circumstances, health behaviours and access to preventive care play a part, but there is also evidence of direct neurobiological and genetic pathways. Cognitive reserve—a concept denoting the resilience of neural networks to age-related pathology—is strengthened by intellectual stimulation, physical activity and social interaction across the life course. Conversely, early adversities such as low birth weight or undernutrition may set individuals on trajectories of cognitive vulnerability, raising subsequent risks of cognitive impairment and dementia. Lifespan studies increasingly employ advanced statistical techniques, including structural equation modelling and survival analysis, to disentangle direct and indirect effects of cognitive ability on health.
