Summary
Temporal lobe epilepsy (TLE) is associated with a spectrum of cognitive and social dysfunction that extends beyond recurrent seizures. Memory impairments, particularly in episodic and verbal domains, are among the earliest and most pervasive deficits, reflecting hippocampal and adjacent cortical pathology. Executive functions—such as attention regulation, planning and cognitive flexibility—are disrupted through alterations in frontotemporal networks. Language disturbances, including word-finding difficulties and semantic retrieval deficits, are often lateralisation-specific, with left-sided foci impairing verbal fluency and right-sided foci affecting prosodic comprehension. Social cognition is also compromised in TLE, manifesting as impaired facial emotion recognition, reduced affective prosody decoding and deficits in theory of mind, which diminish interpersonal engagement and overall quality of life. Surgical interventions, such as amygdalohippocampectomy, can further modulate these impairments, producing material-specific changes in non-verbal and verbal processing. Variability in seizure onset age, lesion type and pharmacoresistance contributes to the heterogeneity of cognitive and social outcomes. Elucidating these patterns and their neural underpinnings is essential for personalised rehabilitation strategies that enhance functional independence and social participation globally.

Recent studies have explored dynamic assessments of social cognition in TLE. One report demonstrated that patients with TLE exhibit significant deficits in affective prosody recognition alongside trends towards impaired performance on a morphing facial emotion recognition task. Crucially, recognition rates across auditory and visual modalities were highly correlated, indicating modality-independent social-cognitive disturbances. These findings emphasise the value of dynamic, ecologically valid paradigms to capture real-world social challenges in TLE.
