Standardised clinical metrics such as the Mini-Mental State Examination and the Clinical Dementia Rating scale permit longitudinal tracking of decline and inform prognosis. Genetic factors, notably the APOE ε4 allele, together with vascular health, lifestyle and comorbid conditions, shape the heterogeneity of trajectories. Recent efforts aim to delineate subgroups with distinct progression profiles, enhance predictive modelling and tailor therapeutic trials to individual rates of decline. A refined understanding of these trajectories is essential for patient counselling, care planning and the development of interventions designed to slow cognitive deterioration and maintain quality of life.
