Cognitive Enhancement Strategies in Neurodegenerative Disease Management

Summary

Neurodegenerative disorders such as Alzheimer’s disease, Parkinson’s disease and related proteinopathies impose a mounting burden on global health through progressive loss of memory, executive function and activities of daily living. Cognitive enhancement strategies aim to bolster neural resilience, preserve synaptic integrity and slow decline by targeting diverse mechanistic pathways. Pharmacological approaches include administration of neurotrophic factors to augment synaptic plasticity, small-molecule agents to modulate neuroinflammation and neurotransmitter systems, and advanced formulations to improve blood–brain barrier penetration. Non-pharmacological interventions encompass non-invasive brain stimulation, structured physical exercise, dietary modulation and cognitive training, each shown to reinforce cognitive reserve and rehabilitative potential. Current research emphasises multimodal regimens integrating molecular therapies with lifestyle modifications, underpinned by biomarker-driven patient stratification and digital monitoring. Ageing populations worldwide accentuate the urgency of efficacious, accessible interventions capable of delivering sustained cognitive benefits across diverse care settings. Together, these strategies hold promise for delaying onset of dementia, enhancing quality of life and reducing societal costs.

Recent studies have demonstrated the potential of systemic delivery of the longevity-associated factor klotho to enhance cognition in aged mammalian models. Low-dose administration of this factor was found to increase synaptic plasticity in rodents and to improve memory performance in aged nonhuman primates without adverse effects. Mechanistic analyses revealed enhanced dendritic spine density, upregulation of synaptic function markers, and improved neuronal survival in hippocampal circuits. These findings offer proof-of-concept for translation of neurotrophic factor supplementation into therapeutic regimens for age-related cognitive impairment and highlight dose-dependent optimisation as critical for efficacy.
