Cognitive Function and Executive Control in Parkinson's Disease
Summary
Parkinson’s disease (PD) is characterised by progressive dopaminergic neuron loss in the substantia nigra, giving rise not only to motor symptoms but also to impairments in higher-order cognition. Executive control encompasses a suite of processes—including planning, attentional set-shifting, inhibitory control and working memory—that support goal-directed behaviour. These functions rely on cortico-basal ganglia-thalamocortical circuits, particularly connections between the dorsolateral prefrontal cortex and the striatum. Disruption of dopaminergic, cholinergic and other neurotransmitter systems within these networks leads to slowed information processing, reduced mental flexibility and diminished capacity to maintain and manipulate information. Neuroimaging has revealed cortical thinning and altered functional connectivity in frontal and parietal regions, while neurophysiological recordings point to early changes in prefrontal firing patterns. Together, these findings emphasise the need to disentangle motor and cognitive deficits in PD and to develop therapies that preserve executive control alongside motor function.

Advanced robotic paradigms have illuminated the interplay between sensorimotor and cognitive systems in PD. In a task involving a robotic manipulandum with virtual obstacles, patients were required to plan and adjust upper-limb movements under dynamic conditions. Off medication, participants were markedly slower, hit more obstacles and showed less efficient trajectories, reflecting combined motor and executive deficiencies. Surprisingly, following dopaminergic medication, sensory interpretation and cognitive performance worsened under complex scenarios, suggesting that restoration of dopamine may differentially influence distinct neural circuits governing sensory integration and high-order control. This work underscores the complexity of executive dysfunction in PD and advocates for assessment tools that separately probe motor execution and cognitive planning.
