Summary
Temporal lobe epilepsy (TLE) is a prevalent focal epilepsy syndrome characterised by recurrent seizures arising in the temporal lobe, most often implicating the hippocampus and surrounding mesial structures. Beyond seizure control, the condition is frequently complicated by deficits in learning, memory, language and executive function, reflecting the pivotal role of temporal networks in cognition. Neuropsychological profiles in TLE are heterogeneous, ranging from isolated domain impairment to global cognitive decline. Contributing factors include lesion type (for example hippocampal sclerosis), disease duration, seizure frequency and interictal discharges, as well as antiepileptic drug load. Emerging evidence highlights alterations in adult hippocampal neurogenesis, network reorganisation and neuroinflammatory processes as drivers of progressive cognitive deficits. Patient‐reported outcomes often diverge from objective tests, underlining the importance of assessing both subjective memory and mood. Recent efforts to classify cognitive phenotypes promise a more tailored approach to rehabilitation and prognostication, with implications for surgical planning, cognitive remediation and quality of life improvement.
