Summary

Instrumental activities of daily living (IADLs) encompass complex tasks—such as managing medications, handling finances, shopping and using transport—that extend beyond basic self‐care and are essential for independent living. Performance of IADLs relies on multiple cognitive domains, notably memory, attention, executive functions and visuospatial processing. In ageing populations and in conditions such as mild cognitive impairment or early dementia, subtle deficits in these domains often manifest first as difficulty with IADLs. Research indicates that declines in executive functions and episodic memory are particularly predictive of reduced IADL capacity and may signal impending progression to more severe neurocognitive disorders. Neural correlates identified through neuroimaging studies highlight the role of frontoparietal network integrity and medial temporal lobe structures in sustaining everyday functional competence. Early and accurate detection of IADL impairment is therefore crucial for timely interventions—ranging from cognitive training and lifestyle modification to assistive technologies—with the ultimate aim of preserving autonomy and quality of life in older adults worldwide.
