Summary
As populations age worldwide, a growing body of evidence highlights a bidirectional relationship between sleep disturbance and cognitive decline. Poor sleep quality—manifested as fragmented sleep, changes in sleep architecture and altered circadian rhythms—undermines processes vital for memory consolidation and brain homeostasis. In particular, reductions in slow-wave sleep and disruptions of rapid eye movement (REM) sleep have been linked to impaired clearance of metabolic by-products and altered synaptic plasticity. Conversely, emerging cognitive impairment can further degrade sleep regulation through neurodegenerative changes in cholinergic and circadian networks. Together, these interacting pathways accelerate progression from subjective memory complaints through mild cognitive impairment (MCI) to dementia. From a global health perspective, understanding this interplay is urgent: the proportion of people aged over 65 is rising sharply, and even modest improvements in sleep across midlife could yield substantial public-health gains in delaying or reducing the burden of neurocognitive disorders. Practical applications include routine sleep screening in memory clinics, chronotherapy to recalibrate biological clocks and targeted behavioural or pharmacological interventions designed to preserve cognitive function by restoring sleep quality.
