Summary

Alzheimer’s disease is a progressive neurodegenerative disorder characterised by an insidious decline in cognitive abilities that ultimately impairs daily functioning. Early deficits commonly affect episodic memory, while language, visuospatial skills and executive control become increasingly compromised as pathology advances. Underlying these clinical features are hallmarks of extracellular amyloid-β plaques, intracellular tau tangles and synaptic loss, which first emerge in medial temporal structures such as the hippocampus before spreading to association cortices. Mild cognitive impairment (MCI) represents an intermediate state in which objective cognitive decline is detectable yet individuals maintain independence in most activities. Semantic memory and word‐finding difficulties often herald progression from MCI to frank dementia, and are reflected in reduced fluency and naming performance. Modern approaches emphasise multidomain assessment—including behavioural testing, electrophysiology and network modelling—to characterise the trajectory from subtle cognitive change to widespread impairment. Such integrative strategies not only enhance early diagnosis but also inform the design of targeted interventions aimed at slowing or ameliorating cognitive decline.

Recent studies have demonstrated that intensive language‐based training in individuals at the preclinical stage of dementia can yield measurable improvements in word retrieval and fluency. A six‐week ‘fill-in-the-blanks’ programme using newspaper and editorial texts produced significant gains in confrontation naming and both semantic and phonemic fluency, with high levels of participant satisfaction. These findings support the feasibility of scalable, content-driven interventions to bolster linguistic function and potentially delay progression to overt dementia.
