Summary
Cognitive impairment in cancer survivorship encompasses a spectrum of deficits in memory, attention, processing speed and executive function that arise during or after treatment. While often referred to colloquially as “chemobrain”, these difficulties reflect a complex interplay of direct neurotoxic effects of chemotherapy, neuroinflammation, vascular and white-matter changes, and oxidative stress. Psychological factors such as anxiety, depression and fatigue, along with genetic predispositions and older age, further modulate risk. Survivors may experience transient or persistent symptoms that hinder work performance, social engagement and overall quality of life. Advances in neuroimaging and blood-based biomarkers have begun to clarify mechanisms and identify at-risk individuals. Interventions ranging from cognitive training and physical exercise to pharmacological modulation of inflammatory pathways are under evaluation. As the global population of cancer survivors grows, systematic screening, personalised rehabilitation strategies and mechanistic research are essential to mitigate long-term cognitive sequelae and to integrate supportive care into standard oncology practice.
