Cognitive Impairment in Parkinson's Disease
Summary
Cognitive impairment in Parkinson’s disease (PD) encompasses a spectrum of non-motor manifestations that range from subtle deficits in executive and visuospatial functions to overt dementia. Mild cognitive impairment in PD (PD-MCI) may arise early in the disease trajectory, often preceding or accompanying motor symptoms. Its heterogeneous profile reflects involvement of multiple neurotransmitter systems, including dopaminergic, cholinergic and noradrenergic circuits, as well as the deposition of pathological proteins such as α-synuclein, β-amyloid and tau. Clinically, PD-MCI carries an elevated risk of progression to Parkinson’s disease dementia (PDD), which affects up to 80 per cent of patients after 15–20 years of disease. The onset, severity and rate of cognitive decline vary widely among individuals, influenced by factors such as age at onset, educational attainment, genetic variants and comorbidities. Neuroimaging studies have revealed structural and functional alterations in frontostriatal and parietal networks, while neuropsychological assessments identify discrete cognitive phenotypes, including amnestic, dysexecutive and mixed subtypes. Early identification of patients at high risk of dementia conversion is critical for stratifying trial cohorts and tailoring interventions aimed at preserving cognitive function. Interdisciplinary approaches combining clinical assessment, biomarkers and advanced computational modelling are driving efforts to refine prognostic tools and to develop targeted therapies that may slow or prevent cognitive deterioration in PD.

Large-scale neuropsychological surveys have quantified the prevalence and heterogeneity of PD-MCI in community and clinic cohorts. One foundational study applied rigorous diagnostic criteria to an Australian sample and found that nearly two thirds of participants met criteria for PD-MCI at baseline, with multiple-domain impairment far more common than single-domain deficits. This work highlighted the predominance of amnestic and non-amnestic multi-domain profiles, underscoring the need for standardised subtyping to guide longitudinal studies. The findings emphasise that PD-MCI is not a unitary entity but comprises diverse cognitive at-risk profiles, each with distinct trajectories and potential therapeutic targets.
