Summary
Cognitive insight refers to an individual’s capacity to reflect on and revise distorted beliefs and interpretations of experiences. Distinct from clinical insight, which denotes awareness of illness and need for treatment, cognitive insight emphasises metacognitive processes such as self‐reflectiveness (the willingness to question one’s views) and self‐certainty (overconfidence in one’s judgments). Impairments in these domains are common across schizophrenia spectrum disorders and have been linked to treatment nonadherence, greater symptom severity and poorer functional outcome. Advances in neuroimaging, network analysis and psychometric validation are converging to offer a more integrative understanding of how disturbances in self‐monitoring and belief‐updating underlie core features of psychosis. Improved characterisation of cognitive insight holds promise for tailoring psychotherapy and cognitive remediation to enhance metacognitive flexibility and, ultimately, functional recovery.
