Summary

Delusional beliefs represent a core feature of psychotic disorders, characterised by fixed false convictions that persist despite counter‐evidence. Cognitive neuropsychiatry seeks to explain these phenomena through the interplay of aberrant perceptual processing, disrupted belief‐updating mechanisms and impaired reasoning. Contemporary models propose that anomalous experiences—arising from altered prediction‐error signalling or dysfunctional salience attribution—provide the raw material for delusional ideation. A secondary disturbance in inferential processing or belief evaluation then allows these explanations to become entrenched. Advances in neuroimaging and computational psychiatry have revealed how dysregulated dopaminergic pathways, particularly within mesolimbic circuits, modulate both the salience of incoming stimuli and the subsequent integration of information into coherent world models. At the same time, phenomenological investigations highlight how personal history, sociocultural context and affective states shape the content and emotional resonance of delusions. This interdisciplinary framework underpins efforts to stratify patients by symptom dimensions, refine mechanistic hypotheses and develop targeted interventions that address both neural dysfunction and lived experience.

By mapping these symptom topographies, researchers are now linking specific clusters to underlying neural circuitry and neurochemical signatures, offering a route towards more personalised diagnostic and therapeutic strategies.
