Congenital Portosystemic Shunt Disorders
Summary
Congenital portosystemic shunt disorders are rare vascular malformations in which blood from the portal vein bypasses the liver and enters the systemic circulation directly. These anomalies arise during embryonic development of the portal and systemic venous systems and are broadly classified as intrahepatic or extrahepatic shunts, depending on whether the anomalous vessel traverses the liver parenchyma. The diversion of portal blood impairs hepatic perfusion and first-pass metabolism, leading to accumulation of gut-derived toxins and altered hormone clearance. Clinically, patients may present at any age with a spectrum of manifestations, including benign and malignant hepatic nodules, hepatic encephalopathy, pulmonary hypertension, neurocognitive dysfunction, endocrine disturbances such as hyperinsulinaemic hypoglycaemia, and growth abnormalities. Prenatal detection by Doppler ultrasound is increasingly reported, although many cases remain asymptomatic until later in life. Diagnosis relies on imaging modalities—ultrasonography, computed tomography and magnetic resonance imaging—often supplemented by liver histopathology. Management strategies range from conservative observation, particularly for small intrahepatic shunts that may close spontaneously, to interventional radiological embolisation or surgical ligation for persistent or extrahepatic connections. A multidisciplinary approach involving hepatology, interventional radiology, surgery and endocrinology is essential. Early recognition and timely closure significantly improve prognosis, prevent long-term complications and offer insights into hepatic vascular biology and systemic metabolic regulation.
