Continuity of Care in Primary Health Systems

Continuity of care in primary health systems refers to the sustained and coherent delivery of health services to individuals and populations over time. It encompasses three interrelated dimensions: relational continuity, which denotes an ongoing therapeutic relationship between patient and clinician; informational continuity, the seamless flow and utilisation of patient data across encounters; and managerial continuity, the coordinated planning and execution of care activities. Evidence indicates that robust continuity is associated with improved health outcomes, greater patient satisfaction and reduced hospital admissions, particularly for chronic conditions and ambulatory care sensitive diseases. Sustainable models often combine team-based practice, integrated electronic health records and patient-centred medical homes. Globally, continuity is challenged by workforce turnover, fragmentation of services and uneven adoption of digital tools. Strategies to enhance continuity include assignment of named general practitioners, structured care pathways for long-term conditions, expanded access programmes and interoperability standards for health information systems. Practical applications range from personalised care plans for multimorbidity to telehealth follow-ups that bridge gaps between consultations.
