Contrast-Enhanced Ultrasound Applications in Hepatic Imaging
Summary
Contrast-enhanced ultrasound (CEUS) has emerged as a versatile, real-time imaging modality for the evaluation of focal liver lesions, vascular architecture, and treatment response in hepatic disease. By administering gas-filled microbubbles that remain strictly intravascular, CEUS affords dynamic assessment of tumour perfusion from the arterial through the late phases without ionising radiation or nephrotoxic contrast. Its high temporal resolution enables precise characterisation of arterial hyperenhancement patterns, wash-out kinetics and tissue stiffness when combined with shear wave elastography. CEUS supports non-invasive differentiation of hepatocellular carcinoma, intrahepatic cholangiocarcinoma and benign nodules, guides percutaneous ablation and biopsy, and permits early evaluation of residual disease after locoregional therapies. Integration into standardised reporting systems and its favourable safety profile have driven global adoption, improving diagnostic confidence, patient stratification and cost-effectiveness in hepatic practice.

A comprehensive analysis investigated how cellular differentiation and tumour size influence CEUS enhancement patterns in hepatocellular carcinoma. In a large cohort of surgically confirmed cases, poorly differentiated tumours and lesions larger than 3 cm showed more inhomogeneous arterial enhancement and earlier wash-out, while well differentiated and smaller lesions exhibited delayed or absent wash-out. Quantitative measurement of wash-in and wash-out times revealed a gradation correlating with histological grade, suggesting that CEUS dynamic parameters can serve as non-invasive biomarkers of tumour biology. Importantly, underlying hepatic background did not significantly alter enhancement kinetics, underscoring the robustness of CEUS for characterising HCC across varying parenchymal conditions.
