Summary
The therapeutic landscape for Covid-19 has evolved from primarily supportive care to a multifaceted approach encompassing antiviral agents, host-directed therapies, immunomodulators and monoclonal antibodies. Early in the pandemic, management focused on respiratory support and prevention of secondary infections. Subsequent research identified repurposed antivirals that inhibit viral replication, notably nucleotide analogues, which shortened time to clinical improvement in non-ventilated patients. Immunomodulatory drugs, including corticosteroids and interleukin-6 inhibitors, have reduced mortality in patients with hyperinflammation by dampening excessive cytokine release. Monoclonal antibodies designed to neutralise the spike protein have been deployed prophylactically and in early disease to prevent progression in high-risk outpatients. Host-directed therapies targeting host pathways required for viral entry and replication have shown promise in early-phase trials. Overall, treatment advances have led to reduced rates of intensive care admission, mechanical ventilation and death, although efficacy varies by disease stage, viral variant and host factors such as age and comorbidity.

Recent studies have demonstrated that outpatient administration of an intravenous antiviral accelerates improvement in biomarkers associated with Covid-19 severity. Treated individuals exhibited faster declines in markers of endothelial activation and coagulation, including soluble angiopoietin-2 and D-dimer, alongside a more rapid normalisation of lymphocyte counts and neutrophil-to-lymphocyte ratio. These biomarker improvements correlated with lower rates of hospitalisation and disease progression, supporting early antiviral intervention in high-risk, non-hospitalised patients.
