Summary
Cutaneous tuberculosis represents a rare but clinically significant subset of extrapulmonary tuberculosis, accounting for approximately 1–2 per cent of all cases. Its epidemiology reflects both the global distribution of Mycobacterium tuberculosis complex and local factors such as HIV prevalence, socioeconomic disadvantage and regional health infrastructure. Clinical presentation varies from paucibacillary forms such as lupus vulgaris and tuberculids to multibacillary manifestations like scrofuloderma and gummatous disease. Routes of infection include direct inoculation, contiguous spread from underlying structures and haematogenous dissemination. Diagnosis is challenging owing to low bacterial load in lesions, overlapping dermatological features and variable sensitivity of traditional methods such as smear microscopy and culture. Advances in molecular detection—polymerase chain reaction and interferon-gamma release assays—have improved case confirmation, while histopathology remains central to classification of granulomatous changes. Management follows standard antitubercular chemotherapy (isoniazid, rifampicin, ethambutol, pyrazinamide) with treatment duration tailored by clinical form and drug-resistance profile. Surgical intervention may be required for extensive tissue destruction. Enhanced surveillance, improved access to diagnostics and integration with broader tuberculosis control programmes are essential to reduce morbidity and ensure timely therapy.
