Cytokine Interactions in Neuroendocrine Regulation
Summary
Cytokines function as pivotal messengers between the immune and neuroendocrine systems, coordinating responses to infection, stress and injury. Proinflammatory mediators such as interleukin-1, interleukin-6 and tumour necrosis factor act on the hypothalamus to modulate corticotrophin-releasing hormone secretion, thereby influencing the hypothalamus–pituitary–adrenal axis. Conversely, glucocorticoid hormones released by the adrenal cortex exert feedback control on immune cells, dampening cytokine release and restoring homeostasis. At the cellular level, microglia and astrocytes in the central nervous system not only produce cytokines but also mediate synaptic plasticity through a tripartite synapse, integrating metabolic, hormonal and immune signals. Dysregulation of these pathways underlies chronic inflammation, behavioural changes and neurodegenerative processes. Recent advances have revealed novel endocrine circuits in which cytokines directly modulate pituitary and adrenal function beyond classical axes, and have highlighted the role of neuroimmune synapses in the fine-tuning of stress resilience. These interactions bear significant implications for the treatment of autoimmune disorders, depression and chronic pain, offering opportunities for targeted modulation of cytokine-driven pathways to restore systemic balance and protect neural integrity.

Recent studies have elucidated the balance between cytokine release and HPA axis activity in human disease. In a cohort of patients with coronary artery disease, acute physical exercise provoked a disproportionate rise in interleukin-6 without corresponding activation of adrenocorticotropic hormone or cortisol, indicating a relative hypoactivity of glucocorticoid feedback in inflamed vascular tissue. These findings underscore the potential of IL-6 as both a biomarker and a therapeutic target for restoring neuroendocrine equilibrium in cardiovascular pathology.

Tripartite synapse: A synaptic structure comprising a pre-synaptic neuron, a post-synaptic neuron and an astrocyte that modulates neurotransmitter and cytokine signalling.
Astrocyte: A glial cell type in the central nervous system that supports neurons, regulates neurotransmitter levels and participates in immune signalling.
