Cytokine Modulation Strategies in Severe COVID-19 Management

Summary

Severe COVID-19 is often driven by an uncontrolled hyperinflammatory response, colloquially termed a cytokine storm, which can precipitate acute respiratory distress syndrome and multiorgan failure. Therapeutic strategies aim to restore immunological balance by targeting key pro-inflammatory mediators and their signalling pathways. Approaches include direct blockade of interleukin receptors, inhibition of downstream signalling kinases, biomarker-guided administration of immunomodulators and, in selected cases, extracorporeal removal of circulating cytokines. Precision in patient selection and timing of intervention are critical: early identification of those at high risk of progression using soluble biomarkers can optimise outcomes and limit unnecessary immunosuppression. Globally, these strategies have become integral to standard care, substantially reducing mortality and the burden on critical-care resources. Ongoing research continues to refine dosing regimens, combination approaches and novel modalities such as cell-based therapies, with the ultimate goal of tailoring treatment to individual inflammatory profiles while minimising adverse effects.

Early intervention guided by elevated soluble urokinase plasminogen activator receptor (suPAR) levels has demonstrated that blocking interleukin-1 signalling with anakinra significantly improves clinical outcomes. In a randomised, double-blind trial, patients at high risk of respiratory deterioration received anakinra alongside standard care, showing faster improvement in a widely used clinical progression scale, reduced organ-failure scores and shorter hospital stays. Mortality at 28 days was nearly halved compared with placebo, underscoring the value of biomarker-driven immunomodulation in preventing severe progression.
