Dacryops and Lacrimal Gland Cyst Management
Summary
Dacryops, a lacrimal gland ductal cyst, presents as a well-circumscribed fluid-filled lesion arising from the orbital or palpebral lobes of the lacrimal gland. These cysts may remain asymptomatic or, when enlarging, produce eyelid swelling, proptosis, restricted ocular motility and even diplopia. Pathogenesis is associated with ductal obstruction, chronic inflammation or minor trauma leading to retention of lacrimal secretions. Diagnostic work-up typically combines slit-lamp biomicroscopy with high-resolution ultrasonography or magnetic resonance imaging to characterise size, location and relationship to adjacent structures. Management ranges from conservative observation of small, stable cysts to definitive surgical excision. Surgical strategies include transconjunctival and transcutaneous approaches, each with advantages in exposure, cosmesis and complication profile. Adjunctive techniques such as laser-assisted fenestration and image-guided planning have been explored to reduce tissue disruption and recurrence. Optimal outcomes depend on precise preoperative assessment, meticulous excision of the cyst wall and preservation of normal lacrimal drainage function.
