Damage Control Orthopedics in Polytrauma Patients

Damage Control Orthopaedics (DCO) has evolved as a pragmatic, staged approach to fracture management in patients suffering multiple life‐threatening injuries. In this model, provisional stabilisation of major long‐bone and pelvic fractures—often via external fixation—permits urgent restoration of physiology before undertaking definitive internal fixation. This contrasts with Early Total Care (ETC), which seeks prompt anatomical repair but can precipitate a “second hit” inflammatory response if applied when the patient remains physiologically unstable. Key to DCO is a dynamic assessment of haemodynamic status, metabolic markers (for example, lactate and base excess), and recognised “windows of opportunity” for safe surgery. By limiting surgical load—defined by intraoperative blood loss, soft tissue damage and operative duration—DCO aims to reduce pulmonary complications, sepsis, multi‐organ failure and overall mortality. Advances in resuscitation protocols, monitoring techniques and imaging have refined indications for each stage of care. Today, DCO principles underpin global trauma guidelines and inform tailored algorithms that balance injury severity, patient physiology and resource availability to optimise outcomes in polytraumatised individuals.
