Summary
Delays in the diagnosis and treatment of pulmonary tuberculosis remain a global health challenge, perpetuating transmission, increasing morbidity and mortality, and undermining control efforts. Such delays encompass the interval from symptom onset to first health-care contact, the subsequent interval to confirmed diagnosis, and the period from diagnosis to treatment initiation. Multiple factors drive these delays, including limited awareness of tuberculosis symptoms, stigma, socioeconomic constraints, fragmented care pathways and variable access to diagnostic tools. Health-system factors such as diagnostic capacity, referral networks and administrative barriers further extend time to care. The cumulative effect of these delays is substantial: advanced disease at presentation, prolonged infectivity in communities and compromised treatment outcomes. Addressing each phase—patient recognition, health-system responsiveness and treatment commencement—is essential for reducing the global burden of tuberculosis and achieving timely intervention.
