Dental Implantology and Peri-Implant Health
Summary
Dental implantology has transformed oral rehabilitation by providing reliable anchorage for prosthetic restorations through the process of osseointegration, in which titanium or ceramic fixtures integrate with peri-implant bone. Success depends on both surgical precision and long-term maintenance of peri-implant tissues. Peri-implant health is characterised by stable marginal bone levels, absence of inflammation in peri-implant mucosa and a balanced microbial ecosystem. Disruption of this equilibrium can lead to peri-implant mucositis—an initial, reversible inflammation of soft tissues—and, if untreated, progression to peri-implantitis, with accompanying bone loss. Key determinants of peri-implant health include implant surface characteristics, surgical technique, host factors (such as smoking, systemic disease and oral hygiene), and the composition of the peri-implant biofilm. Contemporary management emphasises risk assessment, regular monitoring of probing depths and bleeding on probing, mechanical and adjunctive debridement, and, where necessary, surgical intervention. Innovations in guided surgery, antimicrobial therapies and microbiome profiling are reshaping prevention and treatment strategies, thereby enhancing global access to predictable, long-lasting implant therapy.

A comprehensive analysis of the subgingival microbiome across healthy, mucositis and peri-implantitis sites has revealed a stepwise increase in microbial diversity as disease advances. Early peri-implant mucositis is marked by the emergence of classical periodontal pathogens clustering in complex networks, indicating an ecological shift from commensal to dysbiotic communities. In peri-implantitis, the microbial assemblage is further enriched by species such as Eubacterium minutum, which co-occurs with Prevotella intermedia, suggesting synergistic interactions that drive bone-loss pathology. These insights underline the importance of early microbial monitoring and targeted modulation of biofilm composition to halt disease progression.
